By signing this document, you waive certain legal rights, including the right to sue or claim compensation following an accident.

In this Agreement:
“OMLA” includes the Ontario Masters Lacrosse Association, its affiliated leagues and teams, their directors, officers, employees, volunteers, coaches, instructors, trainers, managers, referees, officials, agents, independent contractors, sponsors, facility operators, representatives, successors, and assigns (collectively, the “Releasees”).
“Sport Activities” includes all activities, events, or services provided, arranged, organized, conducted, sponsored, or authorized by the Releasees and includes, but is not limited to:
running; jumping; physical contact; collisions; throwing, passing, hitting or shooting a lacrosse ball; use of a lacrosse stick or other equipment; instructional sessions; practices; games; tournaments; travel; transportation; courses; meetings; warm-ups; conditioning; and any other activities connected to lacrosse participation.
“Equipment” includes all lacrosse or safety equipment provided, loaned, or rented.
I acknowledge that:
I have been advised to wear all required protective equipment in accordance with OMLA and governing-body rules.
Participation in lacrosse requires significant physical exertion that may activate or aggravate existing injuries, medical conditions, or congenital defects.
I have been advised to seek medical advice if I know or suspect that my physical condition may be incompatible with participation.
I will remove myself from participation if I feel unsafe, unwell, or injured.
I acknowledge that lacrosse involves inherent risks, dangers, and hazards, including but not limited to:
Loss of balance or control
Collision with other participants, officials, or equipment
Being struck by a stick or ball
Equipment failure
Uneven or changing playing conditions
Weather-related hazards
Physical contact inherent to sport
Travel to and from activity locations
Negligence of other participants
NEGLIGENCE ON THE PART OF THE RELEASEES, including their failure to:
safeguard or protect me from risks,
provide proper instructions, guidance, or warnings,
maintain facilities or equipment,
meet any standard of care, including duties owed under the Occupiers’ Liability Act (Ontario).
I freely accept and fully assume all such risks, including the possibility of injury, illness, disability, death, property damage, or loss resulting from my participation.
In consideration for the Releasees permitting my participation in Sport Activities and use of their facilities, services, and equipment, I agree as follows:
I waive any and all claims I have or may in the future have against the Releasees and
I release the Releasees from any and all liability for any loss, damage, injury, or expense
(including death) that I or my next of kin may suffer due to any cause whatsoever, including:
Negligence,
Breach of contract,
Breach of statutory duty, including duties under the Occupiers’ Liability Act,
The failure of the Releasees to protect me from risks, dangers, or hazards,
Any negligence relating to the design, manufacture, maintenance, inspection, instruction, or use of Equipment.
I agree to hold harmless and indemnify the Releasees from any liability for property damage, loss, or personal injury to any third party resulting from my participation or use of Equipment.
This Agreement is binding on my heirs, next of kin, executors, administrators, and assigns in the event of my death or incapacity.
This Agreement shall be governed by and interpreted in accordance with the laws of the Province of Ontario, and any litigation involving the parties shall be brought exclusively in Ontario courts.
In entering this Agreement, I am not relying on any oral or written representations or statements made by the Releasees concerning safety, risk, or any other matter, except what is set forth in this document.

This document is being electronically signed under Ontario’s Electronic Commerce Act, 2000.
The electronic signature has the same legal effect as a handwritten signature.
This waiver applies for the full lacrosse season and any OMLA-sanctioned event during the same calendar year.
